DoubleAA has some great advice. However, I would like to throw one more thought your way. Spine angle retention. You must maintain the spine angle you created at address, throughout the entire golf swing. Any straightening up of that spine will cause your arms to raise, which causes your hands to raise, which causes your club to raise.

Like many others i was always led to believe in the straight back and through putting motion. This is not true! The putting stroke, like the full swing, is an up and down motion. Get the putter head working more up and down as opposed to long and low. Also, most people struggle with the application of force. Your putter head should not travel more than 6 to inches past the moment of impact. Work on this and your stroke WILL improve.

Over is right about the posture. Look in a mirror from down the line. Make sure you balls of feet, tips of knees, tips of elbows and back of shoulder joint are all in a line. These are the 4 key checkpoints to setup and posture.
